Parents' Financial Statement (PFS)
The PFS hard copy form for tuition assistance applications is available only from the Live Oak business office. A PFS may be completed online at School and Student Service for Financial Aid (SSS). For more information visit SSS's web site or contact them at 800-344-8328.

Application Checklist
A complete application to Live Oak School consists of the following:
- Completed application form
- Application fee
- Parent Interview
- Student Visit/Assessment
- Completed Teacher Recommendation Form
- Official transcript from student’s current school, including standardized test scores (ERB or STAR) – for applicants to grades 1–8 (Info Release Form here)
- Independent School Entrance Exam (ISEE) – 6th grade applicants only
